Registration

Please note that a pre-registration is mandatory . This can be done by telephone on +49 36702 28 00 or by post.

Prescription

It is important that the diagnosis or the reason for the new prescription of an eye prosthesis is entered by the doctor on the prescription!

A prescription is only valid for 28 days!

Copay

All insured persons of the statutory health insurance funds must pay an additional payment of 10 euros per medical aid according to § 33SGB V.

The following are exempt from this rule: Children and adolescents up to 18 years of age, pension office (war-disabled), employers’ liability insurance associations (BG, eye removal caused by occupational accident!), accident insurance funds and privately insured persons and persons exempt from the additional payment.

Liability

If, as a result of force majeure, illness, accident or the like, it is not possible to carry out the consultation day as indicated, the company cannot assume any liability.
